Options for Submitting Forms/Medical Record Requests:

1. Drop off in-person during business hours.
   
2. Fax to us at 425-338-4366.
   
3. Mail to us at:
   • 15808 Mill Creek Blvd Ste 201, Mill Creek, WA 98012
4. Send electronically via HIPAA-secure link:
   
   • Copy/paste the following link into your browser (not your e-mail): 
     
     • Sendsafe.to/northsoundpediatrics@live.com  
       
       • Input personal e-mail address in the "From" box.
         
       • Attach form(s).
         
       • Add message to body of e-mail.
         
       • Click "Send."

Please note: 

1. WA state law allows up to 15 business days to respond to medical record requests. 
   
2. A signed/current Medical Release Form must be on file in order to release any medical information, including immunization records.
   
3. When receiving a secured message from our office, you will receive TWO separate e-mails. The first will state that you have received an encrypted message from North Sound Pediatrics and the second will provide an access code.
